Community Wellbeing Team

This nurse led service offers a health and wellbeing assessment to those who are registered with a GP in Southampton City.  Click here for more information.

The service is based at Newtown Clinic which is located in Southampton, close to the Royal South Hants Hospital

What can I expect from the Service?

 

An appointment can be arranged at your convenience in our outpatient clinics or in your own home. This will involve an assessment to identify and discuss your Health & Wellbeing needs.

 

As a team of healthcare professionals, we also offer other services as detailed below and work closely with voluntary organisations.  

  • Fracture Liaison Service (FLS)

  • Serious Mental Illness Health Checks

  • ECGs

  • Flu/Covid and Pneumo Vaccinations

Your Appointment

You will be contacted by one of our nurses to arrange an appointment to either visit you at home or to attend a clinic at Newtown Clinic.

 

During this appointment the nurse will carry out a health and wellbeing assessment.

​

You will be provided with health and lifestyle advice, and details of other services locally that may be able to help you.

​

The nurse will liaise with your GP regarding any care provided.

​

If you wish to cancel your appointment, please call 02380 296070 so that this can be rescheduled or offered to another patient.

Parking

​

There is no parking at Newtown Clinic but there are two car parks on the Royal South Hants Hospital site that are within a short walk to Newtown Clinic, which can be accessed via the pedestrian gate (with steps).

​

Please note that parking charges apply in these car parks.

 

Alternatively, there are some free parking spaces on Lyon Street and surrounding roads, but these are limited to 2 hours.

​

There are several bus operators around the City Centre. Please see the bus operator website for specific timetable information or click the link on the map.
